Your first session is a comprehensive consultation where our clinician reviews your complete medication list, recent lab work, and current symptoms. We prepare a medication reconciliation report and share our recommendations with your care team within one business day.

We contact your prescribing physicians directly with our findings. Any recommended changes are discussed and agreed upon collaboratively before implementation. You are kept informed at every step.

Patient Education Session

We walk through your updated medication plan with you in plain language — what each medication is for, how and when to take it, what to expect, and what to watch for. We also provide a written medication summary you can keep at home.

Ongoing Quarterly Reviews

Follow-up sessions are typically scheduled quarterly or as needed based on medication changes or new diagnoses. Between reviews, our team is available by phone if questions or concerns arise.
