What to bring and what to expect
Bring with you
- Driver's license
- Glasses, contacts or hearing aids if you use them
- A list of your medications and doses
- Records for any condition the examiner may need to review:
- Sleep apnea: your CPAP usage report
- Diabetes: a recent A1C result and your provider's name
- Heart conditions: a recent note from your cardiologist
At your visit
- You fill out the driver health history form.
- We check your vision, hearing, blood pressure and a urine sample.
- The examiner does a physical exam and reviews your history.
- If you qualify, you leave with your medical card.
How long is the card good for? Up to 24 months. Some conditions, such as high blood pressure, can mean a shorter card and an earlier recheck. Bringing your records helps you avoid a return visit.
One local clinic for your drivers' physicals and drug tests
We work with trucking companies, contractors, landscapers, bus operators and other employers with commercial drivers. Send your drivers for DOT physicals and drug test collections in one stop.
- Fleet pricing for drivers
- Monthly invoicing instead of driver-paid visits
- Walk-in access, so drivers lose less time off the road
- Reminders before medical cards expire, only if offered
